What to Bring

• All medical and vision insurance cards
• Your driver’s license or photo ID
• Any important medical information or prior eye test results
• An updated list of your current medications

Arrival Time

If you are a new patient, please arrive at least 15 minutes before your scheduled appointment to complete any necessary paperwork.

Cancellation Policy

If you need to cancel or reschedule your appointment, we kindly ask for at least 24 hours’ notice.
